On a virtual box running OpenSolaris snv_134 I reset the publisher to
the internal dev repo and tried to create a new BE based on snv_139.
The first time I tried the image-update almost completed (The Update
Phase was done) but the whole system froze up and I ended up having to
"power off" the vb session. I rebooted in snv_134 again and tried the
image-update again. What I saw was:

$ pfexec pkg image-update --be-name solarisnext_139
DOWNLOAD                                  PKGS       FILES    XFER (MB)
Completed                              636/636 16393/16393  305.1/305.1

PHASE                                        ACTIONS
Removal Phase                              9249/9249
Install Phase                              8261/8261
Update Phase                             22658/22658
pkg: image-update failed: cannot release un-aquired lock

Any thoughts on how I can fix this?

You can ignore the un-quired lock message. It shouldn't affect whether the actual image-update succeeded:
