The main difference is that on Med, High and Very High there are shadows cast by objects such as telegraph lines and stuff, and supposedly nicer trees and grass - although i dont really notice much difference on them.
If you put it on Low, you will notice there will be shadows on cars and stuff - just blobs and rough shapes, AKA Vice city style :P the main difference between VC and SA are the shadows.
