The "Custom Icons" folder is created by InterMapper, so you shouldn't need to re-create it. It lives within the "InterMapper Settings" folder. The location of that directory varies from platform to platform:
Classic Mac: Within the InterMapper application folder
Mac OS X: "/Library/Application Support/InterMapper Settings/"
Windows: "C:\Program Files\InterMapper\InterMapper Settings"
Unix, etc: Varies. Usually within the home directory of the user that intermapperd runs as.
