So this bad decision is to have a standard that everybody is free to ignore?

It means that we change whatever extensions aren't studlyCaps to studlyCaps (except for MySQLi, i.e. this is why it's a bad decision) and make sure in future we stick to CODING_STANDARDS.
I already said that personally I'd prefer for MySQLi to change too.
