I would suggest DHTML or SVG. In either case you use layers (NN) or absolute
positioning (IE). You can build a set of markup tools and use drag and drop
to position them on the page. Click on a tool like a circle or an X and drag
it to where you want on the document. With IE you can then save the file as
a .MHT file which flattens all the graphics into a single file. We have done
this with DHTML. I assume it can also be done with SVG but I do not know
how.

There are other approaches that can be taken if the graphics are of a
standard size and the areas to be marked up are always in the same
locations. You can use the graphic as the background of a table and populate
the surface of the table with transparent graphics which can be changed to a
colored version using onClick or onDblClick, etc.
