I find it hard to believe there's something wrong with the bootloader.

You should try to do a configuration wipe and reinstall the app firmware on
it. It is possible that something got corrupted during previous attempts to
upgrade and this should get everything back to a clean state.

As a last resort, you can use the other IOIO with IOIOManager to reflash a
bootloader on the problematic one.
