For starters, IOIOLib v4.x or v5.x won't work with firmware v3.x. Either upgrade your firmware or downgrade your software. However, the logs you sent suggest that it hasn't even gotten to that stage.
Let's separate some variables here. Can you pick up the pre-compiled HelloIOIO.apk from the compatible software bundle (v3.x unless you upgrade the firmware) and verify that it works?
