OK...
So with Android 4.3 you should definitely run the IOIO with USB debugging
turned off on the phone (to run in OpenAccessory mode).
As soon as you connect the IOIO, regardless of which apps you have
installed (if at all) on the phone, it should bring up the accessory
dialog, which it obviously doesn't.
I've never seen the warning screen you're getting. It may be a
non-standard-Android thing, either added by Samsung or a carrier.
We can attempt the following:

   - Figure out which firmware version you're running on the IOIO. If it's
   ancient, upgrading might help. Note that for the upgrade you'll have to get
   your hands on an *older* Android device, since the IOIO v1 bootloader
   does not (and cannot be made to) work with Android >= 4.1 (this is BTW, why
   the IOIO-OTG uses an entirely different bootloader, which works with a PC
   instead). An alternative is to use a PIC programmer if you happen to have
   access to one. To find out which firmware versions your IOIO has, you would
   need to find a phone that works with it (then run HelloIOIO and look at the
   Android logs) OR get a Bluetooth dongle and attempt to use the IOIO over
   Bluetooth for this purpose. Another thing you might try is to check with
   whomever sold you the IOIO what firmware is on it. This version (v1) has
   been off production for over a year by the major manufacturers, so it is
   very possible that it simply has firmware that predates support for newer
   Android devices.
   - If one of the phones you've mentioned is one you don't mind messing
   with, you can try to install a different ROM on it, for example Cyanogen.

I'm sorry you're having to go through this and I hope I can help, although
there's only so much I can do if this is a problem specific to this phone
model(s).
