First things to check:

   - Are you getting the charging icon on the phone when plugging in the
   IOIO? If not, check your power supply, that the trim pot on the IOIO is
   fully clockwise and that you're using the correct OTG cable supplied with
   the IOIO (or otherwise try to put the host mode switch in the "H" position).
   -  Are you getting the accessory dialog to show up when the IOIO is
   plugged in. If not, you might have forgotten to turn USB debugging off.
