I rolled back to 5.03 and I still get the same errors so it's definitely 
not an issue with 5.04.  Then, it might have been introduced by the latest 
ADT or SDK updates (see my configuration below).

Notice that these errors are thrown only if the device is able to connect 
to the IOIO.  Apps no using the IOIO api, don't throw these errors.  I also 
noticed that onStop() is called, but my overridden ioio::disconnected() 
method is never called so the ioio thread seems to stay alive while the 
main app thread is gone.
