If this is an Android >= 4.2.2:

   - Make sure USB debugging is *off*. Does the Open Accessory dialog
   pop-up when the IOIO is connected?

Otherwise:

   - Make sure USB debugging is *on*. Does the USB debugging icon pop-up
   when the IOIO is connected?

Keep in mind that this is independent of the application.
