The fast blinking LED means that the oscillator has not been tuned, and in order for it to be you need to connect the IOIO to a PC, which may or may not be electrically possible on your custom board (since you removed some of the stuff that's required for USB device). You can bypass the tuning process on the bootloader if you want.
If you're already past the bootloader and have a running app I don't know what the problem might be. Try carefully comparing the USB-related signals between the IOIO-OTG and your board.
