Yes, it seems a simple matter, technically, for iOS to perform an NTP check 
fairly infrequently.

Unlikely, since you can disable this function.

Same here. Perhaps the networks intentionally randomize the time a bit to avoid 
some time-based operations from swamping e network, for instance, every hour on 
the hour?
