Er, are you adding the name again in the address field? Are you talking about 
adding addresses on your computer in OE or in iOS?

When I click “add address” in iOS I get a label (defaults to home) and then I 
have a field for street, and then a field for city, state, zip, and country.

The fields for city and star appear next to each other.

I find by far the easiest way to manage addresses is on the computer, but your 
Outlook Express is very  much out of date. Version 6, the most recent, was 
released in 2001. It’s had a couple of updates since then, but still, it’s a 
decade old. I’m not sure what the best solution for syncing from your computer 
to the iPhone is. It sounds like you are running Windows XP? No, that can’t be 
right, you have a 4S which is iOS 5 which requires at least Vista or OS X 10.7. 
You couldn’t be running OE for Mac on Lion (it’s not Intel native), and OE 
under Vista is, as I recall, unsupported.

I think the real answer is you need a more recent mail client rather than 
running something that is from, or nearly from, last century.
