There are some known bugs with forms on iUI - you must be hitting one of
them. Here's some things to check:
1) What are you doing differently than the samples? (get vs. post,
check capitalization, etc.)
2) Check recent posts on this list
3) See the issues DB, including:

Fixing form issues is a major focus of the forthcoming 0.40 release.
