You just tell phonegap the URL of your PHP web app.

If you are doing your app logic exclusively in PHP though, I would
personally just keep it simple and just
use a UIWebView which is basically a drag and drop and 1-2 lines of
code.

There are examples on how to do this on the iPhone Developer Center
website
