#4 Controlling access will depend upon the server technology that you
are using.
If you want to do the "live streaming" protocol (which works for on
demand, too) you'll need to segment your files. This Ars Technica
article talks about a "beta" of Apple's Segmenter Software which is
something you may want to investigate:
