I've created new pull request (it fact - updated my previous pull request).
The source file and changes are available at

It mostly devoted to using puzzles in IKE_SA_INIT exchange.
I tried to describe it in details, however a few sections
are left blank for now as they are more or less obvious.
This pull request also includes the section describing DoS
protection measures after IKE SA is created and
a short section proposed by Yaron on using resumption
in case of DoS attack. There is also a section describing
the proposed format for the PUZZLE notification.
