I had completely misunderstood what the person I was replying to meant. There was a third option which Fernando put admirably clearly later, i.e., simply that the beginning of the ESP header had to be in the first fragment. How much of the beginning of te ESP header needs to be present in the first fragment is, as you pointed out, a good question. The fragment header, if present, is always the last thing in the non-fragmentable part. I was imagining a scenario where this was no longer the case.
