Charlie, If you have Microsoft Excel or any other spreadsheet program, you can easily make forms like that. You can make a wide margin on one side if you want to punch them and put them in a binder too. I have done that in the past before I decided to give up on paper whenever I could. Of course, when you're making them, they will look better on a plasma monitor :) Mike Hawkins
