My guess would be that the requestTimeout option is leaving the connection in a 
state that conflicts with the state of the FailoverTransport.  I would not 
advise using request timeout in combination with the failover transport, or I'd 
treat any exception coming from the NMS API in that case as fatal and tear down 
and rebuild the connection.
