Missing user_authentication_id_seq in ambari_sequences table upon upgrade.
The missing sequence should be added using a value greater than the largest
{{user_authentication.user_authentication_id}}. For example:
{code:sql}
insert into ambari_sequences (sequence_name, sequence_value)
select 'user_authentication_id_seq', max(user_authentication_id) + 1 from
user_authentication;{code}
