Review Comment:
   Fixed — read_map now tracks a cumulative `items_read` counter (incremented 
by each block's count) and uses it for the limit check instead of 
`len(read_items)`, so duplicate keys can no longer undercount and bypass the 
cap. Added a duplicate-key regression test.
