If a JdbcSort has a RelFieldCollation.direction that is different from the JDBC
source's default direction, we should generate NULLS FIRST or NULLS LAST.
If the source does not support that syntax, devise something else; e.g.
{code}ORDER BY ISNULL(x), x{code} in MySQL; see
http://jira.pentaho.com/browse/MONDRIAN-226.
The dialect should know what each JDBC source's default direction is.
