In looking into some issues reported by [~onpduo], we both noticed that
{{FetchRequest.offset}} is never actually used by the server.
We should make sure the semantics on this value are clear (is it an offset from
the beginning of the ResultSet or the current position of the ResultSet?) and
make sure it is used.
