Support CREATE TABLE in tests. It is useful to be able to create tables with 
small amounts of test data for the duration of tests. We should add {{CREATE 
TABLE}} the same way that we added {{ALTER SYSTEM UPLOAD JAR}} in CALCITE-1384. 

Also, make it available to Quidem tests via a connection specification and 
schema called BLANK.
