SimpleCxfRsBinding should:
- ensure the Camel In Message body is null when the body index is -1 and it is
not multipart - the default binding which SimpleCxfRsBinding extends still sets
the body to a CXF specific response even if no body is available - which causes
side-effects when a RestDsl binding is enabled.
- should work if a CXF RS model is used as opposed to the interface
