CLOUDSTACK-3560: UI createForm: Support multiple dependsOn targets
Support passing list of dependsOn targets for form fields. This will
trigger a change if any items specified in list are changed.
Example:
fieldA: {...},
fieldB: {...},
fieldC: { dependsOn: ['fieldA', 'fieldB'] }
