For Hyper-V 2012 R2, we need to create an external vswitch manually before 
adding the host to CloudStack. This needs to be documented.
This can be done from UI (provided the host is added to any Hyper-V manager)
In the manager, click on the host -> Virtual Switch Manager -> New Virtual 
Switch -> External network (choose the NIC adapter) -> Apply

This is in contrast to Hyper-V running on Windows 2012 R2, where it gets 
created automatically.
