I've found that where we used to enter *.mydomain.com in 
consoleproxy.url.domain it changed to just mydomain.com a little while back.  I 
changed my url back to *.mydomain.com, restarted mgmt service and console proxy 
started working again.

The code in core/src/com/cloud/info/ConsoleProxyInfo.java has been reverted 
either deliberately or accidentally.
