Reproduction:
1. Create a managed storage
2. Create Volume on the managed storage
3. Attach the volume to a VM
4. Stop the VM and remove the host the VM was running on
5. Restart management server
6. Adding new hosts will fail because SolidFire will try to send a modify
storage command to removed host and it fails
