improve FileUploadLimitExceededException metadata

Hi,
It would be very helpful if FileSizeLimitExceededException stored the uploaded
file name and html field name as properties.
This is helpful data to display in an error message especially if there are
multiple FileUpload fields. An example message could be:
The file 'MyDoc.pdf' is too large. The 'Upload CV' field is limited to 1mb.
