Committed (with minor additional Javadoc fixes) in revision 1164474.

Leaving open until confirmation that {{ConvergenceException}} is the right one 
to use. I thought that we could make a difference between _theoretical_ and 
_implementation_ convergence failures. But it might not be worth introducing 
the distinction just for this one case, especially since it is quite clear 
clear now that the class should not be used.
