CSVParser uses CSVFormat.getSkipHeaderRecord to initialize index mapping and
skip the first record as well (initializeHeader method). It's inconsistent that
CSVPrinter doesn't skip creation of header record since the skipHeaderRecord
boolean should be applicable for reads/writes.
