I believe I'm seeing the same issue, however mine only occurs on the initial
run just after app installation. The second run works just fine. I'm still
investigating if it could be due to something else like the device ready event
not firing, etc.

*UPDATE*: Upgrading to Cordova iOS 4.1.0 fixed the issue. Mine was actually
