I have figured out a workaround based on an article found here:
http://twit88.com/blog/2008/05/14/net-mtom-enabled-your-application-using-wse/
Doing this in .NET got it working:
Finally, change the class to inherit from
'Microsoft.Web.Services3.WebServicesClientProtocol' instead of
'System.Web.Services.Protocols.SoapHttpClientProtocol'.
Now .NET does receive the SOAPFault with MTOM enabled or disabled on the
response.
