If a message results in a fault due to some issue with the WS-Addressing
related headers (for example: OnlyNonAnonymousAddressSupported), the resulting
soap:Fault is correct in the body, but there are not addressing headers present
on the fault (such as a "RelatesTo" header and FaultDetail headers.
