Description: Setting the unique-constraints for the index values in orm.xml
is causing problems for some databases. Instead we can set unique=true on the
OpenJPA Index annotations. (was: Even though the default is "unique=false" for
the OpenJPA Index values, it seems to be causing a problem with some DBs by not
setting it explicitly.)
