Opened #1342 implementing the lineage-based identity we converged on above: root events keyed by (businessKeyIdentity, seqNum), output events keyed by (producing action execution's own composite key, output ordinal), folded together with the existing attributes hash rather than replacing it so the content-based replay-divergence protection stays intact. Covers the recovery cases from the design discussion (yield/resume continuation, completed-action replay, unfinished-action re-execution) and adds a regression test for the original sibling-collision repro plus a full operator-level fan-out test.
