[~vnick] and [~hoanbc], I believe this is a known issue in the current
implementation of "keyboard interactive" authentication for SSH (see
GUACAMOLE-141). The various OTP solutions that work with SSH tend to leverage
"keyboard interactive", however Guacamole's implementation is incomplete and
assumes that it will be used only as a single prompt (for a password).
