Add per region request information to HServerLoad

HBASE-3507 added per region request count.
We should utilize this information so that HServerLoad can provide moving
average of request counts to load balancer.
We can update this method in HRegionServer:

private HServerLoad buildServerLoad() {
{code}
The above method can aggregate request counts from HRegions and store it in
HServerLoad.RegionLoad
