Block cache reference should not be static

After the introduction of CacheConfig, over in HBASE-4641 we dealt with a bug
of the block cache being instantiated in the master. A short-term fix was done
there.
This is the real fix. The block cache should not be a static reference, it
should be instantiated within HRS construction/initialization.
