HBASE-13477 Create metrics on failed requests
Summary: Add metrics on how many requests are exceptions and what type.

Nothing on why. I'd imagine that when we are throwing these if lots of clients,
yeah, we could be making loads of them.
Its nice being able to see what queue is full and by how much.
Let me leave this patch here until gets more petitioners.
