OK, the clearDeadServers call is triggered by user...

And for removeServers, we are only allowed to remove servers which are not in 
online servers and recorded dead servers.

At first this looks a bit strange, as it seems that clearDeadServers is enough. 
But the problem here is that, after master restarts, the dead servers record 
will be cleared, so the current approach is correct I think. But we need to add 
more comments for this.
