Consistently include column family / qualifier info in audit logs.
    
This patch makes the AccessController consistently add information about column 
families and qualifiers into audit logs. Since there may be more than one cf / 
qualifier involved, the format of the audit log message changed slightly 
(coupled with some pretty ugly code to create the message).
    
TestAccessController passes; also tested on live HBase 0.92 (with a backported 
version of the patch).
