How does this work for IO elevator thread, and other potential permanent
threads (e.g. when thread runner will be changed to be on a threadpool and not
on separate threads)?
I had problem with MDC (iirc) in that the MDC would be set once for a
threadpool thread and then all the fragments would log that MDC.
