Thanks for the taking a look , i think thatss the root cause, but it looks
like `createForEntireSegment` and `IndexSearcher.search` pass `NO_MORE_DOCS` on
purpose.
Also, `BulkScorer` do not require max <= maxDoc, so other callers can may
hit the same path (not sure if this holds).
Happy to change those call sites to pass maxDoc as well, and keep the clamp
here as a safeguard.
Does this make sense as long term solution?
