here's the output of the fork. The footer in this case was modified from a two
column table row to a three column table row which allows for
header
- chapter, header, page
footer
- centered footer
I'm not how use desirable my modifications are for users of the pdf plugin but
making these changes were none trivial and are opt in settings.
