Actually the DOXIA-138 discussion doesn't make sense to me. What I meant by
"Confluence would do it" was that if you had a Confluence wiki then it would
automatically render anchors on its section headings. Why would that be a
concern purely for the output format when Doxia has an "anchor" event as a
first-class citizen?
