A page head that declares `charset`, `viewport` or `generator` produced a second element of each. The skin's `charset` (from the output encoding) stays the only one; a page's `viewport` or `generator` replaces the skin's. New IT `mskins-323` uses an xdoc page with its own head. Fixes the skin part of #323.
